Pool Access Request

Request pool and bathroom access, a household key fob, and/or mobile app access. This form will be submitted to Inframark upon completion. Please watch for email correspondence or phone calls from Inframark.

Access rules and deadlines

Please complete this form in its entirety. You will receive an email from Avigilon Alta (noreply@alta.avigilon.com) to download the Mobile Access APP to obtain access to amenities once this form is received and processed by Inframark. Your HOA account must be in good standing (zero balance) to obtain access. If you lease the home, a current lease agreement will be required with a copy of your driver's license with this form before the email link is issued. After you submit this online form, it will be sent for review. If Inframark needs additional documents, such as a lease agreement or identification, they may contact you directly.

Requests for additional or replacement fobs require payment through the Inframark homeowner portal. Residents are also asked to pre-register for the portal. Fob pickup details will be emailed later with a future day and time for pickup at the pool.

Each household is allowed one free key fob.

One additional household key fob may be requested for a $50 fee.

Replacement key fobs cost $50 each.

HOA account must be in good standing with a zero balance before access is issued.

Tavola West CAI Amenities Release & Indemnity

I/We the undersigned homeowners residing at ____________________________________________ , do hereby accept responsibility for a secured access card to the swimming pool area and other associated facilities situated in the Tavola West Community Association Inc. subdivision. By acceptance of this access, and in consideration of access and use of the Pool, we hereby agree to:

1) INDEMNIFY, and HOLD HARMLESS Tavola West, and its' agents, directors, officers, employees, and contractors from any and all liabilities, costs, claims, damages, and suits arising, in whole or in part, out of the actions of, or in the use of the Pool/Rec Center by, the undersigned, by any family member of the undersigned, any resident of the address shown above, or by any guest of any family member or resident.

2) RELEASE Tavola West and its' agents, directors, officers, employees and contractors, on behalf of ourselves and any person or entity for whom the undersigned have legal authority to do so, from any claim arising in whole or in part out of the actions of any third party occurring on or within the premises of the Pool.

3) RELEASE TAVOLA WEST COMMUNITY ASSOCIATION, INC., AND ITS' AGENTS, DIRECTORS, OFFICERS, EMPLOYEES AND CONTRACTORS; ON BEHALF OF OURSELVES AND ANY OTHER PERSON OR ENTITY FOR WHOM THE UNDERSIGNED ARE LEGALLY AUTHORIZED TO GRANT A RELEASE, FROM ANY CAUSE OF ACTION ARISING IN WHOLE OR IN PART FROM THE NEGLIGENCE OF TAVOLA WEST COMMUNITY ASSOCIATION, INC. OR ITS' AGENTS, DIRECTORS, OFFICERS, EMPLOYEES, AND CONTRACTORS IN THE MAINTENANCE OR OPERATION OF THE POOL/FACILITIES; AND

4) INDEMNIFY AND HOLD HARMLESS TAVOLA WEST COMMUNITY ASSOCIATION, INC. AND ITS' AGENTS, DIRECTORS, OFFICERS, EMPLOYEES, AND CONTRACTORS AGAINST ANY CLAIM BROUGHT BY ANY FAMILY MEMBER OF THE UNDERSIGNED, ANY RESIDENT OF THE ADDRESS SHOWN ABOVE, OR BY ANY GUEST OF ANY FAMILY MEMBER OR RESIDENT, ARISING IN WHOLE OR IN PART FROM THE NEGLIGENCE OF TAVOLA WEST COMMUNITY ASSOCIATION, INC. OR ITS' AGENTS, DIRECTORS, EMPLOYEES, AND CONTRACTORS IN THE MAINTENANCE OR OPERATION OF THE POOL/ FACILITIES.

The Releases and Indemnification provided for herein includes, but is not limited to, any liabilities, cost, claims, damages, and suits arising out of falls, slipping, accidental drowning or any other such incidents resulting in injuries or damages to ourselves or any third party.
